Once a suspected security breach has been properly reported, the Merchant must complete the Post-Reporting Incident Response Form. This form will ensure that all parties are aware of the breach and assist the Bank Card Committee in continiually improving Western's Security Breach Plan.
The Merchant must forward this form to the PCI Working Group (instructions are located on the form).
